Document

Every volunteer must sign the attached waiver before volunteering. Please print, fill out and bring with you on your first day. For those over 18, pages 1-4 are applicable. Please include an emergency contact in the designated space, even as an adult. Minors must sign pages 1-3, skip 4, and then complete page 5 with their parents/guardians.

If you have no way to print out the waiver, we will have copies on-site you can fill out as well.
